Saunders Medical Center Care Center is licensed as a 60-bed long term care skilled nursing facility and offers a comprehensive range of long-term-care services.  Each room has individually controlled heating and air conditioning, television outlet, and telephone jack for a private phone.  We believe that all residents have a right to individualized care and support.  Care must be planned using a multidisciplinary approach integrating medical, nursing, dietary, social work, therapeutic recreation, and family input.  We provide 24-hour professional nursing services.  Our registered dietitian is available to provide for nutritional needs for the residents as ordered by the physician.  When ordered by a physician, we can offer physical therapy, speech therapy, occupational therapy and respiratory therapy.  Daily activities are provided to meet the recreational needs of the residents.  Our social worker serves as an advocate for the residents, staff, and families, and is available for one-to-one family and resident consultation. Our  residents may receive professional hair care service, may have cable television connected in their rooms, may set up personal accounts, and will be provided with opportunities to attend various religious services regularly in our chapel. Both private pay and Medicaid are accepted.

The facility is divided into three neighborhoods which provide a combination of clinical and social care. Each neighborhood has a private area for activities. Residents and their families play a key role in the choices of care based on the resident’s abilities. Our neighborhoods are based on need and include: Countryside - City View - Memory Lane

SMC provides around-the-clock nursing care provided by Registered Nurses (RN), Licensed Practical Nurses, Medication Aides and Certified Nursing Assistants (CNAs).  This provides enhanced continuity of care as staff and residents have more time to interact and get to know one another. The caring and highly-skilled staff have expertise in geriatric care and assure that residents' needs are met.

Additional Services:

·         In addition to a number of indoor areas, all residents have access to patio areas and sidewalks for outdoor activities that residents and families may use.

·         Family and friends of residents are invited to participate in any activities. They are also encouraged to join their resident for occasional meals. In addition, families may arrange to celebrate holidays or have special family events.

·         Activities are designed, coordinated and implemented by the Activities Director, the Nursing staff, and SMC representatives. They are an important aspect of resident care and contribute significantly to the quality of life provided for residents

·         Both female and male residents may receive full-service hair care, including shampoos, shampoos and sets, haircuts, permanents and tints.

·         A stylist is available at the salon on a regular basis. Appointments may be made by contacting her in the Salon or by contacting the LTC Reception Desk.

·         Any licensed stylist in the community is welcome to provide services in the LTC Salon. Residents or their families may arrange to have hair care services provided by residents' "regular" stylists in the Salon by contacting the stylist and then scheduling the appointment at the Reception Desk.

·         To keep family members informed and involved in planning their loved one's care, a Care Planning Conference is scheduled quarterly for each resident and family. Several rooms are available for family activities. Families may reserve these for special celebrations and social events. 

  • Physical Therapy: Physical Therapists provide personalized therapy that assists residents to gain strength and mobility, and to become more independent. Equipment includes parallel walking bars, exercise bicycles, stairs, range of motion wheel, weights, mechanical passive exercise tables, treadmill, a mechanical walking device, ultrasound, hand/foot whirlpool, and other devices.
  • Occupational Therapy: An occupational therapist provides specialized devices and training designed to assist residents in becoming as independent as possible with feeding, dressing, and grooming. Upper arm strengthening, range of motion, gross and fine motor coordination activities, transfer training and a wheel chair seating and positioning evaluation are part of their program development. Also, an in-home safety assessment for those residents planning to be discharged may be ordered by their physician.
  • Speech-Language-Swallow Therapy: The Therapy Department also includes a licensed Speech Therapist. The therapist provides evaluations, therapy and consultations for residents who require treatment of speech, language, cognition, and swallowing disorders.

Restorative Care

Restorative care is a planned, systematic program that focuses on helping each resident obtain and maintain the highest level of function.

·         Focuses on helping residents do things for themselves rather than “doing for“ the resident

·         Focuses on short, achievable goals

·         Focuses on the resident's functional potential, not a diagnosis or limitations

·         Focuses on preventing deterioration whenever possible

·         Seeks to improve the resident's self-image and self-esteem

Examples of goals of restorative care:

·         Promote activity, mobility and independence in ADL's

·         Increase muscle strength and balance

·         Promote continence

·         Prevent contractures and pressure ulcers

The purpose of restorative care nursing is to take an active role in helping residents maintain their highest level of function, thus preventing excess disability. Restorative care is given to an resident after they have had a medical event or need to work muscle groups they do not usually use. More than physical therapy, restorative care is used for emotional well being as well. The goal is to not only help the resident recover from a broken bone or pulled muscle, but also to get the person to socialize and want to be part of a group. Many times, elderly people will not want to participate in social activities because they are not able to move around easily or because they are afraid to make new friends. Provides meaningful activities designed to promote physical, social and emotional well-being. Activities are tailored to the individual needs of each resident and are an important part of his/her plan of care.

VOLUNTEER OPPORTUNITIES in LTC: Spending just a little time with a resident may bring to mind your own treasured memories and create new ones. Your genuine interest in an elderly person’s life during occasional weekdays, evenings, or weekends helps residents feel valued. It brightens their day and improves their overall health. You can help with activities that are fun for residents.Just a few of the many ways you can become involved are listed below.

·         Friends Program: Commit to become a special friend. Give attention at least two times per month; visit or write and show special attention on holidays and birthdays (a family, team, or other group could do this).

·         Entertainment: Provide music programs—instrumental, vocal, and dance.

·         Craft Projects: Participate in planned craft projects or bring suggestions of your own.

·         Parties: Assist with holiday parties, Happy Hour, or other occasional parties.

·         Reading: Mail, newspapers, books, poetry, or Bible stories for individuals or groups. This activity helps stimulate a person’s mind.

·         Games: Bingo, board games, card games, puzzles.

·         Escort: Assist residents during an outing to a community event, to the fair, a parade, fishing, or other outing.

·         Transporter: Assist residents to and from activities and events.

·         Bible Study: Lead Bible study or Hymns during Bible study or church service.
